Mid Sussex Wellbeing

Xyla collaborates with Mid Sussex District Council to deliver a complimentary lifestyle support programme tailored to adults registered with a GP in Mid Sussex. Each programme is customised to suit the individual’s needs. Our evidence-based methodology, coupled with continuous support, encouragement, and motivation, guarantees sustainable changes that endure a lifetime.

How does it work?   

This programme offers remote accessibility, accommodating a flexible approach tailored to your lifestyle. With a blend of one-to-one sessions, group workshops, and online learning modules, you’ll receive comprehensive support throughout your entire journey.  

  • You will have 12 weeks of one-to-one sessions with your Health and Wellbeing Coach   
  • You can choose between having your sessions face-to-face or remote  
  • The programme includes online learning, so you’re fully supported for your whole journey   

   

How can I sign up?   

If you feel that you would benefit from the service, are over 18 years and are registered with a Mid Sussex GP Practice then you can refer yourself below. Alternatively, you can ask your GP, nurse or NHS Health Check professional to refer you.
