Xyla have provided patient centred care coordination and hospital discharge case management for CHC, local authority, and self-funding clients at Kettering General, Northampton General, and surrounding community hospitals since December 2016.

Our approach

The Xyla team build strong relationships with local care providers, giving regular updates on capacity to support collaborative work to achieve efficient discharges into the right place-based care. Our team work closely with system partners to support discharge initiatives, an example of this being the recent focus on ‘Home First’ discharges.  

Xyla have adapted our services to support shorter term projects such as the delirium pathway and ‘Home for Christmas, and we continue to manage Northamptonshire’s non weight bearing patients through their rehabilitation.  

Xyla’s service supported the system with a datadriven approach to give full visibility of the patient journey to our NHS partners. The following results indicate potential efficiency savings for the self-funding patient cohort from December 2016 to January 2022. 

Impact

From December 2016 – January 2022 Xyla have discharged a total of 928 self-funding patients from KGH and NGH Hospitals 

If these patients had been subject to the national average (18 days) this would result in a cost of circa £6,681,600 in bed days. Xyla NET time (median) is 5 days, which would cost circa £1,856,000 in beds days. Therefore, Xyla involvement has generated a potential bed day saving of : £4.8m Financial savings 

Speed of service 5 Days– On average, Xyla were able to discharge these patients from hospital in <5 days. This service speed is from the point of referral to Xyla, to the point of discharge of the patient.
