Registered Nurse – Adult Continence Community Assessments (South West London – Merton)

Position:
Registered Nurse – Telephone Reviews & Community Assessments

Role Overview:
We are seeking a skilled and experienced Registered Nurse to join our Adult Continence Service team in South West London, Merton. This role involves both telephone assessments and face-to-face (F2F) community assessments, providing care and support to patients in their homes.

Duties of the Role:

  • Telephone Appointments (300 patients annually)

    • Conduct telephone assessments and annual pad reviews with patients.
  • F2F New Patient Assessments (150 patients annually)

    • Visit patients in their homes to conduct thorough community assessments.
    • Provide detailed assessments for bladder scanning, catheters, TWOC, and other continence care needs.
    • All necessary equipment will be provided.

Patient Load:

  • Telephone Appointments: 15 minutes per patient
  • F2F Assessments: 1 hour per patient
  • F2F Assessments: 5 patients per day

Experience & Skills Required:

  • Must be a Community Nurse (Band 6) with experience in bladder scanning, catheters, and TWOC procedures.
  • Must have relevant postgraduate diploma/degree or specialist advanced course in Continence Care.
  • Experience in community assessments and using SystmOne or EMIS is essential.

Training Requirements:

  • SystmOne Community Modules (approx. 3 hours virtual)
  • Possible onsite orientation.

Working Hours:

  • Start Time: 08:30
  • End Time: 17:00 (1-hour unpaid lunch break)
  • Working Days: Mondays and Wednesdays (2 days per week)
  • Total working hours per week: 16 hours

Contract Duration: ongoing to meet service needs

Administrative Responsibilities:

  • Complete assessments on patient systems (SystmOne or EMIS).
  • Send letters to patients and GPs.
  • Make referrals as needed.
  • Provide discharge reports.
  • Submit daily service delivery reports and data capture for each day worked.
  • Report on DNAs (Did Not Attend) and urgent patient needs as required.
