Supporting patient flow: Blackpool Teaching Hospitals

Overview

Commissioned to support Blackpool Teaching Hospitals NHS Foundation Trust in alleviating pressures on their hospital discharge process.

The problem

The discharge process fell to their team of eight Complex Case Nurses; taking a significant amount of time and resource that was sorely needed elsewhere. The average time taken to discharge a medically optimised patient was nine days before Xyla were commissioned.

Our solution

  • Xyla receive referrals from Blackpool Borough Council or the CHC team, which covered funding types such as, social services funded, fast tracks, D2A funded and CHC, all on pathway 2 and 3.
  • Xyla support the hospital discharge process in BTHNFT by ensuring a timely and efficient discharge for medically optimised patients, enabling the patient to be placed in the most appropriate setting within the community.
  • We bring flexibility and adapt to the needs of BTHNFT wherever necessary, meaning that we’re able to expand or contract our service as the capacity needs change.

Impact

  • 94% KPI We are exceeding our 80% KPI of all patients, or patients families to be contacted by Xyla on the day or referral.
  • 2 day KPI We are currently exceeding our 5-day KPI, from when patients are medically optimised to time of discharge.
  • 88% rated the Xyla service either excellent or very good
