Overview

Xyla provided patient centred care coordination and case management to give full visibility of the patient journey to the ICB, local authority, and NHS partners 

We worked with the ICB to support the remodelling of the locations and types of beds contracted by the hospital Xyla developed the ongoing diagnostics/solutions based approach to reach optimum flow in and out – using data gathered from ‘CHSTrack’ and our expertise 

Impact

  • Xyla managed 200 beds across 20 locations.
  • Initially commissioned on a cost neutral basis: reduced 82 beds to 62.
  • 29 days Reduction in patients’ length of stay.
  • £1.6m Cost Savings in excess bed days.
  • 60% Patients discharged within 24 hours of referral.
  • 30% Increase in patients returning home after D2A stay.
