Here’s an example of how you could start the conversation with your GP:
“I’d like to be referred to the NHS Type 2 Diabetes Path to Remission programme. It’s delivered by Xyla here in Northamptonshire. I’ve checked the eligibility criteria online and I think I meet them. Could you put the referral in for me?”

Once your GP has completed your referral:

This will be to arrange an initial assessment, talk through the programme in more detail and discuss whether the Total Diet Replacement phase is suitable for you.
The sessions will be with trained diabetes practitioners. You will be supported throughout, working with your practitioners to help make changes that are realistic and sustainable over the 12-month period.
