About the programme

The NHS Type 2 Diabetes Path to Remission programme is a free, NHS‑funded programme designed to support people with Type 2 diabetes to lose weight, improve their health and, for some, put their diabetes into remission. 

Throughout the programme, you’ll be supported by trained diabetes practitioners. Your journey begins with an initial assessment, followed by regular coaching sessions and check‑ins over 12 months. Support is personalised, practical and designed to fit around your lifestyle. 

The programme includes an initial phase using low‑calorie meal replacement products for up to 12 weeks. This stage is fully supported and followed by a gradual reintroduction of everyday foods, along with longer‑term support to help you maintain weight loss and build healthy habits for the future.

Am I likely to be eligible?

You may be suitable for referral if: 

  • You are aged 18 or over 
  • You have Type 2 diabetes 
  • You were diagnosed within the last 6 years 
  • Your BMI is 27+ (or 25+ for people from minority ethnic backgrounds) 
  • You are not currently using insulin 

Your GP practice will confirm eligibility as part of the referral process. 

How to talk to your GP about getting a referral

If you think the programme could be right for you, the next step is to speak to your GP practice. 

You may find it helpful to explain: 

  • When you were diagnosed with Type 2 diabetes 
  • Any weight loss or lifestyle changes you’ve tried before 
  • Any additional conditions or symptoms you feel are linked to your weight  
  • Why you are interested in the NHS Type 2 Diabetes Path to Remission programme and ask whether you meet the criteria for referral  

What happens once I’ve been referred?

Once your GP has completed your referral: 

Step 1

You will receive a welcome email and/or letter telling you more about the programme

Step 2

You will be contacted by the programme provider (Xyla) to arrange an initial assessment, talk through the programme in more detail and discuss whether the Total Diet Replacement phase is suitable for you

Step 3

If suitable, we will book you for your future sessions with your trained diabetes practitioners. You will be supported throughout, working with your trained diabetes practitioners to help make changes that are realistic and sustainable over the 12-month period.

For GPs: How to refer a patient to Xyla

Complete the T2DR referral form found on your clinical system

Include any medication adjustments required as part of the T2DR referral

Submit the referral to the email address on the referral form or via your clinical system
