NHS Sussex ICB needed a more efficient and dependable way to manage the volume and complexity of All Age Continuing Healthcare reporting. Three separate Caretrack systems produced isolated datasets for East Sussex, West Sussex and Brighton and Hove, which meant staff had to manually merge and validate information before it could be used. This created delays and made it difficult to maintain a consistent county wide picture.
The move from the Patient Level Data Set to the All Age Continuing Care Data Set increased the detail required for national submissions. Existing tools could not present information in a clear visual format or support quick access to data when urgent queries arose, which added pressure for operational teams.

To support national reporting, the AACC data set workflow was refined into a single monthly submission. As the tool became established, access was extended so operational teams could use it independently, giving staff simple and reliable visibility of activity and performance.
Regular check ins ensured the tool evolved in line with Sussex’s priorities. Improvements focused on making reporting straightforward and user friendly, and on supporting teams manage AACC demand effectively.
Teams accessed county level information in one place, reducing manual administration and improving data accuracy. This gave the system a dependable reporting rhythm and a clearer view of overall AACC activity.
The consolidated dataset made All Age Continuing Care Data Set submissions quicker and simpler to prepare. Accuracy improved and teams had greater confidence when meeting national deadlines.
Real time dashboards give operational teams clearer insight by highlighting trends and delays earlier. Teams can track shifts in demand more easily and base day‑to‑day decisions on reliable up to date information. The BI tool provides visual graph-based views and quick filtering across longer date ranges or multiple patient cohorts, giving teams fast, flexible access to the insights they need.
Live access to Sussex wide data enabled rapid responses to time sensitive queries from leaders and partners. This improved systemwide responsiveness and reduced the need for manual checking.
Built in dashboards replaced bespoke spreadsheets and manual charts, creating a more consistent approach to reporting. Teams always worked from the latest available information without relying on offline files.
