Discharge to Assess (D2A) – Staffordshire ICB

Published 26 November, 2025

The challenge

When we began working with Staffordshire ICB, the region had 82 spot‑purchased beds that were not being used efficiently. Xyla worked with the ICB to remodel bed locations and types and reduce the number of beds. Using our bespoke database and expertise, we coordinated assessments and admissions to optimise length of stay and improve patient flow, ensuring patients moved smoothly through the system.

Our solution

Reduction in spot-purchased beds

We reduced the total beds from 82 to 62, improving efficiency and ensuring resources were used effectively.

Block bed provision

We introduced block bed provision in local areas, aligned with patient locations, to provide consistent and accessible care.

Optimised bed utilisation

Through patient-centred care coordination and case management, we gave full visibility of the patient journey and ensured beds were allocated and managed effectively.

Cost-neutral service

We reduced reliance on spot-purchased beds and streamlined provision to deliver a cost-neutral service without compromising quality.

Improved patient care

Providing the right beds in the right locations improved patient flow and care outcomes across the region.

Impact

30% increase in patients returning home after D2A stay

Patients discharged within 24 hours of referral

£1.6m cost savings in excess bed days

29 days reduction in patients’ length of stay
