✔ Attend all sessions: We understand that there may be situations beyond your control that result in a missed session. If you cannot attend a session, you must inform us as soon as possible. If you miss multiple sessions, you may be discharged from the programme.
✔ Provide health measurements: For your safety, you will need to provide us with health measurements throughout the programme. If you choose the digital programme, we provide kit for you to take your measurements at home. If you choose the face-to-face programme, your coach will take your measurements during sessions. These measurements include blood glucose levels and weight. For anyone with hypertension, blood pressure will also be required. In the event we are unable to obtain your measurements, we will need to discharge you from the programme as it would be unsafe to continue without them.
✔ Order your Total Diet Replacement (TDR) as instructed: Choose your product flavours carefully, as changes aren’t possible after ordering. If you can’t continue, please dispose of any unused items.
✔ Follow medication guidelines: Only start your Total Diet Replacement meals on the mutually agreed start date (this will take place between your first and second session). Do not change any medications prior to this. If you are on the Digital programme, you are required to use the Wellbeing Way app: Download the app as instructed after your initial assessment and engage in the online modules to stay on track. If you are on the Digital programme, you must use your measuring kit as instructed: Log measurements on the app and have your measurements ready for your sessions.
✔ Keep us informed: Let us know if you wish to withdraw from the programme or if your GP advises you of any further changes to your medication. This is to ensure you are safe on the programme.
✔ Consult your GP if discharged: If you withdraw or are discharged from the programme, you are required to discuss this with your GP as soon as possible. For your safety, Total Diet Replacement meals should not be consumed unless you are medically supervised.
✔ Attend yearly GP reviews: Keep up with your regular diabetes check-ups.
✔ Update us on health changes: Inform us and your GP about any health changes that might affect your participation (e.g., pregnancy, kidney problems, heart issues, etc.).
