How does The NHS Type 2 Diabetes Path to Remission Programme work?

For the first 12 weeks of the programme, you will follow a low-calorie diet (typically shakes and soups). Following this, we will work with you to healthily reintroduce food into your diet, focusing on movement and maintaining a new healthy eating lifestyle.

TDR Phase

You will replace your usual diet with your chosen total diet replacement (TDR) products for 12 weeks. You will also start working with one of our diabetes practitioners on a range of behavioral change sessions.

Food re-introduction phase

Our diabetes practitioners will support you to re-introduce food into your diet in a safe healthy way. This includes helping with meal planning and understanding nutrition and a healthy balanced diet.

Weight maintenance phase

The final phase of the programme will provide you with the knowledge and tools you need to continue to achieve your health and wellbeing goals and hopefully put your type 2 diabetes into remission.

How do we deliver The NHS Type 2 Diabetes Path to Remission Programme?

No matter whether you choose our Face to Face 1-1 or Digital 1-1 programme, we will work together over 12 months to achieve your goals.

Digital sessions

  • One-to-one digital sesseions via phone calls with your Diabetes Practitioner, supported by the Wellbeing Way app
  • Choose a time and day that is best for your practitioner to call you
  • Free measurement kit to give measurements to your practitioner each week
  • Message your Diabetes Practitioner via the wellbeing Way app for support between sessions
  • Appointment length varies throughout the programme with a mix of 15 and 30-minute sessions

Face-to-face sessions

  • One-to-one sessions in person at a location near you
  • Choose a time and day that is best to meet your practitioner face-to-face
  • Have your measurements taken at each session
  • Appointments are 30 minutes long

Please use the email address we have for you on record. Once you log in you can choose the digital or face-to-face programme, choose your preferred sessions and book. You will then receive confirmation on your booking via email or post.

Programme choice: Digital (phone and app) or in-person

Session preference: Your preferred session day (Mon-Fri) and time (morning, afternoon, or evening)

Medications: The names of any medications you are currently taking for diabetes and/or blood pressure and whether you have been advised to make any changes to these when you start the programme.
