Tissue Viability Nurse – Hertfordshire

Position: Tissue Viability Nurse (Domiciliary Visits)
Location: West Hertfordshire
Contract Pattern: Monday to Friday, 8:30am – 5pm
Start Date: 20th January 2025
Contract Duration: 17 weeks

Role Overview
We are seeking a skilled Band 6 Tissue Viability Nurse to join our team in the West Hertfordshire area. This role involves assessing and treating a wide range of wounds in patients’ homes, ensuring they are placed on appropriate treatment plans and discharged to planned care when suitable.

Key Responsibilities
•    Conduct assessments and treat wounds across the body, including Category 3 and 4 wounds.
•    Manage wound care for new and follow-up patients:
o    New patients: Up to 2 per day.
o    Follow-up patients: Up to 3 per day.
•    Use advanced wound care techniques, including PICO devices, Negative Wound Pressure Therapy, and IV antibiotics.
•    Complete admin tasks, including:
•    Documenting in SystmOne after appointments.
•    Booking follow-ups and making onward referrals to planned care.
•    Provide support and education to carers and family members.

Requirements
o    Demonstrated expertise in managing a variety of wounds, including complex cases.
o    Familiarity with PICO devices, IV antibiotics, and Negative Wound Pressure Therapy is preferred.
o    Must be a car driver with a valid UK driving license.
o    Completion of SystmOne training (provided virtually via the trust)
