Community Respiratory Nurse – Wandsworth

Are you an experienced Respiratory Nurse looking for an opportunity to make a real difference in the community? Our client is seeking a skilled and compassionate nurse to join our team, providing essential care to patients in Wandsworth.

Location: Wandsworth area. Base location – SW17

Type of Service: Community-based with domiciliary visits

Contract Type: Monday to Friday only, 9:00 AM – 6:00 PM

Start Date: ASAP

Contract Duration: 7 weeks

The successful applicant must be a car driver

Role Responsibilities:

• Conduct new and follow up assessments for patients with suspected or new respiratory diagnoses (e.g., COPD, Asthma).
• Provide post hospital discharge care and contribute to hospital admission avoidance strategies.
• Perform physical exams, oscillations, NEWS, FeNO tests
• Prescribe and review medications, complete patient notes on EMIS, and send updates to GPs.
• Refer patients to Consultant-led or O2-led MDTs as required.
• Discharge patients with a comprehensive care plan when appropriate.
