The challenge

With ongoing patient flow challenges, Chesterfield Royal NHS Trust faced:

• A shortage of care capacity in the community

• High numbers of patients with no criteria to reside in hospital

• Patients being stepped up into Pathway 2 block bed provision awaiting a package of care

• Unnecessary extended hospital stay causing concern and anxiety to patients and families


Xyla was contracted to implement a uniquely collaborative, patient-centric end-to-end intermediate care service to help alleviate these pressures. Initially contracted to deliver a pilot‑scheme, Chesterfield Royal widened and extended the contract following the excellent results delivered.

Our solution

12-hour HCA provision

  • Care packages delivered between 1–4 visits per day

28-day service model

  • A structured 28-day service with the provision to support onward care requirement

  • Focused activity designed to reduce anxiety and confusion for patients and their families

Through our solution, we helped reduce pressure by rapidly discharging patients, freeing up essential resources and capacity. This created a highly effective collaborative system that improved patient flow across the entire trust.
