Carried out independent Trusted Assessments for both care homes and hospital teams, enabling more efficient placements, reduced assessment waiting times, and timely, safe discharges.

Flexible funding support

Supported a full range of funding arrangements and accepted referrals for County Durham patients in IC+ beds or across neighbouring NHS trusts

Impact

Facilitated 1,040 safe discharges, including Pathway 3 and complex needs patients, into 24 hour care placements, averaging 3 days from referral to discharge

Completed 417 Trusted Assessments within 2 hours, enabling faster discharges

Delivered a time saving of 4,344 hours for Adult Social Care teams. (A total of 91,417 activities were completed)

98% of service users rated our service as excellent/very good
