In 2018: Hillingdon hospital had 149 stranded patients with a total of 3,358 bed days.  

• Lack of capacity with home care providers 

• Waiting for care or nursing home places 

• Out of area placements 

• Family choice delays 

Our solution

20 stranded and super stranded patients were identified and referred to Xyla

Ward closed as a result of the scheme

Length of stay of stranded patients reduced by 377 bed days in two weeks

Equates to 24 beds saved on an annual basis 

From point of referral, with Xyla intensive discharge support, patients were discharged within 7.5days
