Staying on track: Your 12-month journey to success

1. Expect highs & lows: That’s normal!
  • Weight loss is not a straight line. Fluctuations, especially during food reintroduction, are expected.
  • Stay focused on long-term goals. This programme is about lasting change, not just short-term results.
  • Consistency is key. Small, steady changes lead to big results over time.
2. Setting and achieving your weight loss targets
  • You have been referred to this programme for a reason: to improve your health and achieve diabetes remission
  • Your goal is achievable! Break it into smaller, manageable targets over 12 months.
  • Track your progress and celebrate every step forward, even the small wins!
3. Why we move to monthly sessions
  • Helps you become more independent in managing your lifestyle.
  • Gives you time to apply what you have learned and adjust to real-life challenges.
  • Prepares you for maintaining progress after the programme ends.
4. Long-term health: The bigger picture
  • Managing weight helps protect your liver and pancreas, lowering the risk of complications.
  • Sticking with the programme can support remission from type 2 diabetes and better long-term health.
  • Just because things feel good now does not mean they always will. Staying committed helps keep you on track.
5. Why staying committed matters
  • Weight loss is not linear. Your body takes time to adjust, and plateaus are a normal part of the process. Staying consistent is key, and if you are feeling stuck or frustrated, talking to your coach can help you stay motivated and find the right approach for overcoming these hurdles.
  • Knowledge alone is not enough, applying it consistently over time is what leads to lifelong change.
  • Maintaining progress is just as important as achieving it. Staying on the programme helps prevent setbacks and keeps you on track for the long run.
  • If you are used to quick results, it can be tough to trust the process. Real, lasting change takes time. Stick with it, and the results will follow.
  • Motivation comes and goes, but commitment is what leads to success. The programme is here to help you reset, refocus, and keep going when things get tough.
Before you decide to leave, ask yourself:
  • Am I quitting because this truly is not right for me, or because I am facing a challenge I can overcome?
  • Will I regret not following through on something I committed to?
  • How would my future self-feel be looking back at this decision?
  • What does leaving early say about my long-term commitment to my health?

Remember: The people who achieve lasting success are not the ones who never struggle, they are the ones who keep going despite the struggles. If you are feeling discouraged or unsure, your coach is there to help and guide you through the tough moments.

6. Staying the course when it gets tough
  • Gained some weight? This is normal in the transition phase – keep going!
  • Lost motivation? Remind yourself why you started – better health, more energy, and feeling your best.
  • Feeling stuck? Reach out to your coach or group. You are not alone, and we are here to help! Your coach will help you navigate challenges and keep you motivated to stay on course.
Final encouragement: Progress over perfection
  • Weight loss is not an “all or nothing” journey – every step forward counts, even if it is slower than you would like.
  • Setbacks are not failures – they are just part of the process. Learn from them, adapt, and keep going.
  • The programme is here to support you. If you are struggling, reach out. You don’t have to do this alone.

Stay committed. Stay consistent. Stay the course. Your health and wellbeing are worth it – and your future self will thank you!

Final word: Finish what you started

  • You began this journey with a reason – to feel better, live healthier, and take control of your future. That reason still matters.
  • This programme is here because you matter – your health, your goals, your potential.
  • The NHS believes in that. We believe in you.
  • You have gained so much already – new knowledge, support, habits, and confidence. Why not see it through?
  • Completing the programme is a powerful act of self-respect.
  • It shows your family, friends, and GP that you are serious about your health.
  • It is a thank you to the support you have received – and an investment in your future.

Stay the course. Finish strong. You will be proud you did!
