We understand that the NHS Type 2 Diabetes Path to Remission Programme can bring with it unique challenges throughout the 12-month programme. To help support you on the programme, we are pleased to offer you free personalised support from our psychological support service ‘Empower’.
Empower provides psychological support based on Cognitive Behaviour Therapy (CBT) ideas and principles and provides you with the tools to help address, manage and overcome challenges. It is designed to enhance and support your journey on the remission programme.
These short-term psychological sessions provide you with ongoing skills to support you to take control of your thoughts and behaviours, to help you throughout the programme.
Research shows that staying actively involved in your program significantly increases your chances of achieving remission. Our holistic approach combines both mental and physical health support, ensuring you receive comprehensive, person-centered care. We’re dedicated to equipping you with the resources you need to enhance your overall well-being, all delivered in a seamless and accessible manner.
Empower will run alongside your NHS Type 2 Diabetes Path to Remission Programme sessions over 3 weeks. Each week you will take part in a 30-minute sessions.
Appointments are accessible 7 days a week, all delivered one-to-one to focus on your specific needs for supporting remission. You can choose the delivery method that suits you best, including telephone and video options.
No, these Psychology based sessions are designed to complement the NHS Type 2 Diabetes Path to Remission Programme. It works in tandem with the programme to address psychological barriers that may emerge during the process.
This service is free to those on the West Yorkshire NHS Type 2 Diabetes Path to Remission Programme.
The sessions are delivered by Psychological Wellbeing Practitioners who specialise in offering psychological support.
The sessions are designed to explore your goals related to the program and identify ways to support your mental health and wellbeing, enhancing the program’s effectiveness. We use methods to help you understand your mental health and introduce gentle strategies to overcome challenges during the program, incorporating problem-solving and some principles from Cognitive Behaviour Therapy.
No, sessions remain confidential – we would only share information that could result in either risk to self of others.

Appointments are delivered digitally through telephone or video. Sessions are private and delivered on a 1 to 1 basis. Appointments are available 7 days a week.
Sessions are 30mins each week over 3 weeks.
An initial assessment will take place on session 1, followed by 2 x sessions providing psychology-based tools to help support with common challenges faced with the remission programme.
