From echocardiographer to clinical scientist: The AHCS equivalence route  

For highly specialised echocardiographers, the scope of practice has expanded significantly. Advanced reporting, complex valve assessment, service leadership, education and quality oversight are now routine in many roles. In practice, this means a growing number of professionals are already working at a level aligned with clinical scientists, even without formal recognition. 

The Academy for Healthcare Science (AHCS) equivalence route provides a structured way to formally recognise this expertise. For services facing workforce pressure, recruitment challenges and the need for sustainable leadership, equivalence plays an important role in supporting progression, retention and professional clarity. 

What is the AHCS equivalence route?  

The AHCS equivalence route is designed for experienced practitioners who have not completed the Scientist Training Programme (STP), but whose knowledge, skills and experience match the same standard. 

Rather than focusing on qualifications alone, mapping a portfolio to Good Scientific Practice standards (GSP) ensures clinical competence, scientific and analytical judgement, leadership, education and training, and contribution to research or service improvement are also assessed. 

What does equivalence look like in echocardiography?  

For echocardiographers, the evidence submitted often reflects the reality of modern services. This may include independent reporting of complex studies, leadership of specialist clinics, responsibility for service development, delivery of training and mentorship, and active involvement in audit or quality improvement. 

The process is intentionally rigorous and reflective. Applicants must clearly articulate how their experience and day-to day practice matches clinical scientist‑level expectations via a portfolio of evidence matched to GSP and an interview by a panel of experts. While time‑intensive, this reflection often strengthens professional confidence and supports consistent, high‑quality service delivery. 

Successful completion of equivalence leads to eligibility for registration with the Health and Care Professions Council (HCPC). This formal recognition supports career progression and helps services develop advanced and consultant‑level roles.

Below, members of our team share their personal reflections on why they undertook the equivalence process and what it meant for their careers.

Nicole Fadden

I began the AHCS STP equivalence process with a clear ambition to progress towards becoming a Consultant Clinical Scientist by gaining clinical scientist equivalence and then pursuing  HSST‑level doctoral training. Working in a District General Hospital, the challenges around workforce capacity and service sustainability were very real, and I wanted to contribute not only clinically, but through leadership and service development.  

Equivalence allowed me to remain embedded in clinical practice while demonstrating that my experience aligned with the Scientist Training Programme outcomes. Mapping my work against the Good Scientific Practice domains gave me a structured way to assess my development across leadership, education and research, as well as clinical practice. This task allowed me the opportunity to reflect on how my role had evolved, and the impact of each of the projects and responsibilities I had taken on had to our service.  

The process was challenging but ultimately validating. It strengthened my confidence in my role and reinforced my commitment to supporting high quality, sustainable clinical science services. 

Rozanne Moreton 

Much of my career has also been spent in a District General Hospital, where service development and workforce challenges are especially visible. I undertook STP equivalence to demonstrate alignment with the STP MSc curriculum enabling me to develop physiologist‑led services.  

The process improved my confidence significantly. Reflecting on my own leadership, service improvement projects and the support I gave others helped me recognise the breadth of my experience. While gathering evidence across all areas was demanding and the timescales were long, the rigour of the process including the interview made the final outcome extremely meaningful. 

Achieving equivalence was a moment of genuine pride and reinforced the importance of robust professional standards in supporting future service models.

When I started STP equivalence in 2020, it felt like a natural progression. From my traditional route into the career my role had evolved to include advanced echocardiography, leadership, training, quality improvement and research. Equivalence offered a way to formally recognise this scope and create future opportunities. 

Compiling the portfolio required me to think beyond technical competence, focusing on professional judgement, accountability and service impact. Balancing the workload was challenging, but aligning my experience with Good Scientific Practice provided reassurance and validation. 

The process reinforced the value of reflection and continual development, not just as requirements, but as tools that strengthen practice and improve care.

The healthcare science career pathway is becoming more structured, with clearer expectations for advanced roles. The STP equivalence route provides a formal framework for experienced practitioners to demonstrate alignment with the current curriculum. 

I am in the early stages of engaging with the equivalence guidance, not because it is mandatory, but because it is increasingly relevant. Understanding the framework now allows me to lead by example and better support colleagues through mentorship and service leadership. 

Within cardiac physiology, the lack of HCPC recognition has historically limited progression. Equivalence offers a route to formal acknowledgement and the opportunity to elevate the profession, strengthening both individual careers and the services we deliver.
