The NHS Type 2 Path to Remission Programme combines specialised nutrition, psychological support and physical activity to drive rapid weight loss, enable sustainable behaviour change, and support type 2 diabetes remission and reversal.

NHS Digital Adult Weight Management

The NHS Digital Weight Management Programme supports adults living with obesity who also have diabetes or hypertension. It provides a 12-week digital behavioural and lifestyle programme, accessible via smartphone or computer, helping individuals improve their health through structured support and guidance.

Referral into the NHS Digital Adult Weight Management Programme is open to a range of healthcare professionals working across primary and community care settings, such as:

  • GPs
  • Practice Nurses
  • Healthcare assistants (via clinical pathways)
  • Community pharmacists
  • Other primary care clinicians

NHS Behavioural Support for Obesity Prescribing (BSOP)

The NHS Behavioural Support for Obesity Prescribing (BSOP) programme is designed to support individuals who have been prescribed tirzepatide, combining medical treatment with tailored behavioural, nutritional, and psychological support.

Supporting safe and effective weight management pathways

The programme provides structured, person-centred support for patients starting weight loss treatment with pharmacological therapies. It ensures patients receive the wraparound care required to achieve sustainable outcomes and meet QOF expectations.

The programme helps patients to:

  • Build sustainable habits that support long-term weight management and metabolic health
  • Develop balanced dietary behaviours and make informed lifestyle choices around nutrition, physical activity, and self-care
  • Understand obesity as a chronic condition, with tailored strategies to support lasting change
  • Improve confidence, health literacy, and engagement through consistent, non-clinical support alongside prescribing pathways
  • Three flexible ways to take part in the programme: Remote group sessions, In-person sessions, Digital programme

Eligibility Criteria

  • Aged over 18
  • BMI of at least 40 kg/m2 (reduced to 37.5 kg/m2 for people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African Caribbean ethnic backgrounds)
  • 4 or more of the following comorbidities: Dyslipidemia – Hypertension – Cardiovascular disease – Obstructive sleep apnoea – Type 2 Diabetes
