Our operational service solutions

We provide direct, practical support across three critical stages of a patient’s journey: preventing unnecessary hospital admissions, enabling safe and timely discharge, and coordinating pathways and community care. Our teams add capacity where it’s needed most – helping health and social care teams improve flow, reduce length of stay, and achieve safe, person-centred outcomes.

Admission avoidance

  • Pathway 0 support – we provide support to care providers to keep individuals safe and prevent unnecessary hospital admissions.
  • Patient tracking and follow-up – we provide tracking and follow-up for patients, as well as ongoing welfare support to prevent avoidable admissions.
  • Community signposting – we connect individuals to local community services and resources to maintain safety and wellbeing.

Hospital discharge and complex cases

  • Hospital discharge – we support coordination throughout key stages of the discharge process; sourcing care providers quickly, and keeping partners, providers, and families informed every step of the way.
  • Trusted assessor – we carry out independent assessments for care homes using region-specific templates, building strong relationships with ward staff and care providers, including introductions before schemes start to ensure accuracy and timeliness.
  • Super-stranded patients – we support discharge of patients in hospital for 21+ days, resolving barriers with system partners, patients, and families, focusing on timely, safe, and person-centred outcomes.

Pathway coordination and community support

  • Discharge to Assess (D2A) – we coordinate and track discharges across Pathways 1, 2, and 3, managing social care assessments and therapy input to reduce length of stay and ensure smooth transitions to home or community placements.
  • Supported discharge – we provide HCA support on 12-hour shifts with 1–4 daily visits, managing cases end-to-end, communicating with families, updating system partners, and ensuring a safe and swift discharge into the pathway.
  • Surge ward management – we deliver 24/7 nursing and HCA staffing to relieve resource pressures, reviewing patient needs to reduce ongoing Pathway 1 & 2 care requirements and improve flow.
