Right to Choose: Adult ADHD assessments

We offer a comprehensive ADHD assessment service that is available under the NHS Right to Choose pathway. If you and your patient agree that an ADHD assessment is clinically appropriate, they can choose our service as their provider.

Xyla is an acreditted provider, currently accepting referrals for Adult ADHD assessments for patients registered with GPs in Herefordshire and Worcestershire.

Important information: Xyla will begin accepting patient referrals from 24 September 2025. Please do not submit referrals before this date, as they will not be accepted.

How our referral process works

Discuss Right to Choose with your patient

Step 1. Self-report form

Your patient needs to complete a self-report form. If they haven’t done so yet, you can assist them in filling out the digital self-report form during their appointment with you.

Step 2. Formal referral request

Your patient needs to provide you with a formal letter indicating their choice of Xyla as their ADHD care provider and requesting your referral. This simplifies the referral process for you.

Step 3. Refer your patient to Xyla

Once you’ve confirmed your patient meets the eligibility criteria for our Adult ADHD assessment service, please complete a referral using either one of the forms provided above.

  • You can submit the referral online using our digital form, or
  • Download and complete the offline form (check your downloads folder after clicking) and submit it to xyla.rtc@nhs.net.

⚠️ Important: We are currently only accepting referrals for patients registered with a GP in Herefordshire and Worcestershire.

After Xyla receives your patient referral

Step 4. Referral confirmation

Once Xyla receives your patient’s referral, you’ll receive an email confirmation to let you know we’ve received it.

Step 5. Full diagnostic assessment in line with NICE guidance

Our expert clinical team will guide your patient through our thorough online ADHD assessment process, ensuring a detailed evaluation.

Step 6. Medication

Once a patient is diagnosed with ADHD through our service, we offer a full medication pathway in partnership with Boots Digital Health, including initiation, titration, and stabilisation. Our clinical team works closely with patients to monitor progress and adjust treatment as needed. Once stabilised, patients are transitioned back to their GP under a shared care agreement, ensuring safe and consistent ongoing management within primary care.

Important information about NHS Right to Choose

As a responsible healthcare provider, we cannot accept patients who do not meet our suitability criteria. Your patients won’t be able to choose Xyla if:

  • They are receiving urgent or emergency care.
  • They are already getting treatment for the condition you’re being referred for.
  • They are in prison, temporarily released from prison, or in other specific settings (like a court, secure children’s home, secure training centre, immigration removal centre, or a young offender’s institution).
  • They are held in a hospital under the Mental Health Act 1983.
  • They are a serving member of the armed forces (but your family members in England have the same rights as other residents).

If any of these apply to a patient you are referring, they will not be able to directly choose Xyla as your provider at this time.
